Use the checklist to ensure that key elements such as appropriate introductory statements, demographics, and directions are included on each of the tools you develop. The following instrument is a sample and may not contain the key elements specified on the checklist. If you choose to use the sample instrument, Project STAR strongly encourages you to use the checklist to modify the sample to meet your program’s needs.
