The following sample instruments are not intended to be used "as is." You are encouraged to use these instruments as a guide as you develop performance measurement tools that are appropriate to your program. If you would like assistance, Project STAR is available to help.

• Instrument Development Checklist (PDF)*
  This has been developed in an effort to assist you with developing and/or modifying your program instruments. Use the checklist to ensure that key elements such as appropriate introductory statements, demographics, and directions are included on each of the tools you develop. The following instruments are samples and may not contain the key elements specified on the checklist. If you choose to use one of the sample instruments, Project STAR strongly encourages you to use the checklist to modify them to meet your program’s needs.
